Job summary

Oasis Rehabilitation and Nursing is seeking a dedicated Clinical Care Coordinator, RN to support high-quality resident care through close monitoring of resident conditions, timely clinical follow-up, staff education, and collaboration with nursing leadership. This full-time role is ideal for an experienced registered nurse with strong clinical judgment, excellent communication skills, and a passion for serving older adults.

Shift: Monday-Friday. Pay Rate: $58.00 per hour.

Qualifications

• Valid New York State Registered Nurse license required.
• Current CPR certification required.
• Strong clinical assessment and communication skills required.

Responsibilities

• Attend daily morning meetings to receive updates on residents experiencing changes in condition.
• Closely monitor residents with condition changes or stability concerns.
• Review labs, diagnostics and clinical information daily for timely intervention.
• Support triage, assessment and clinical interventions as needed for residents.
• Communicate findings and changes in condition with physicians, residents, families and the interdisciplinary team.
• Collaborate with nursing staff to promote strong clinical communication and continuity of care.
• Round on residents admitted in the last 30 days and those with infectious concerns.
• Ensure clinical documentation is completed to support the resident record.
