Indigo Hall is seeking a dedicated Resident Care Coordinator to support residents' health and well-being by coordinating care services, managing ISPs, and ensuring regulatory compliance. You’ll work closely with the Executive Director, care staff, and healthcare providers to maintain a safe, person-centered environment.

Qualifications

• Requires an Associate’s or Bachelor’s degree in Nursing, Healthcare Administration, or related field.
• Active LPN or RN license preferred (depending on state).
• Minimum of 2 years in senior living, assisted living, memory care, or healthcare setting.
• Previous supervisory or leadership experience is highly desirable.
• Proficiency in electronic health records (EHR) systems and basic computer skills.

Responsibilities

• Develop and manage individualized service plans (ISPs) for residents in collaboration with clinical staff, residents, and families.
• Lead caregiving staff, ensuring adherence to care protocols and staffing levels.
• Serve as primary contact for residents and families regarding care-related concerns.
• Ensure compliance with state regulations and company policies through audits and QA activities.
• Collaborate with nursing, physicians, therapists, and leadership to coordinate care.
