Job summary

Mercy Hospital in Nassau County, part of Catholic Health, is seeking a dedicated care assistant to support residents through life skills training, meal planning, and daily living activities. The role emphasizes person-centered care, documentation, and collaboration with a behavioral specialist to implement individual service plans.

The position requires a High School Diploma or GED (Bachelor’s preferred), valid NYS driver's license, and experience with special needs.

Qualifications

• High School Diploma or GED is required.
• Bachelor’s degree preferred.
• Experience with special needs or mentally disabled adults preferred.
• Valid NYS driver’s license required.

Responsibilities

• Assist in development, delivery and coordination and monitoring of skill training with residents and staff.
• Provide supportive direction in life skills training to residents of the Community Residence.
• Assist with creating environment for development of social skills and utilize resources.
• Assist residents with planning, purchasing and preparing meals; help run home operations.
• Complete documentation and records per guidelines and regulatory requirements.
• Report resident behavior and outcomes in progress notes and logs.
• Ensure accurate billing documentation reflects the individual service plan.

Education

High School Diploma/GED

Bachelor’s degree preferred

Experience with special needs or mentally disabled adults preferred

Maintains valid NYS driver’s license
