Responsibilities

• Serve as a primary point of contact for residents, families, and healthcare providers.
• Facilitate communication between clinical teams, physicians, and support staff.
• Assist with coordinating resident care plans and services.
• Identify resident needs and connect them with appropriate internal or external resources.
• Support smooth transitions of care and continuity between providers.
• Provide education and guidance to residents and families regarding available services.
• Monitor resident progress and communicate updates to the interdisciplinary care team.
