Job summary

Columbia Place Assisted Living & Memory Care is seeking an experienced Resident Care Coordinator (RCC) to join our Health Services leadership team. This hands-on role coordinates caregivers and Med Techs, ensuring high-quality resident care and a well-staffed Health Services department.

You will manage scheduling, participate in on-call rotations, and partner with nursing leadership on resident plans and changes.

Qualifications

• Minimum of 1 year of experience as a caregiver/Personal Care Attendant OR at least 6 months of experience as a Medication Aide/Medication Technician.
• Assisted living experience strongly preferred.
• Previous scheduling, lead, supervisory, or care-coordination experience preferred.
• Strong organizational and communication skills.
• Ability to communicate effectively with residents, families, employees, and leadership.
• Ability to problem-solve and make sound decisions in a fast-paced environment.
• Must maintain required certifications, including medication and CPR certifications as applicable.
• Must be willing and able to provide direct resident care when needed.
• Must be available for scheduled weekends and holidays.
• Must be able to participate in the on-call rotation and respond to staffing needs outside regular business hours.

Responsibilities

• Manage and maintain the caregiver and Medication Technician schedule, including coordinating time off, vacations, call-outs, and staffing changes.
• Ensure staffing levels meet resident needs and state requirements.
• Provide on-call support for Health Services staffing needs and employee questions; participates in on-call rotation.
• Provide leadership and day-to-day support to caregivers and Medication Technicians.
• Assist with interviewing, hiring, onboarding, training, and development of Health Services staff.
• Collaborate with nursing leadership on resident service plans, evaluations, and changes of condition.
• Communicate resident changes and concerns to nurses, providers, families, and others as appropriate.
• Assist with medication-room and cart organization, supplies, and equipment.
• Respond to resident emergencies and follow up as needed.
• Serve as liaison between frontline Health Services staff and community leadership.
• Help maintain infection-control, safety, and resident-care procedures.
• Occasionally work as Medication Technician or caregiver to support the department and cover shifts.
• Assist with additional Health Services responsibilities as assigned.
