Qualifications

Job description

Schedule: Full-time - Full-time/dayshift (Monday – Friday)

Location: 100 Heritage Lane. Albany, NY 12211

The employee in this position is a Registered Professional Nurse at the Residential Health Care Facility who is responsible for ensuring completion of Minimum Data Set (MDS) assessments and related documents including Care Area Assessments (CAAs) and Comprehensive Care Plans. The incumbent directs and supervises Resident Care Coordinators. The work is performed under the general direction of the Director of Nursing. Does related work as required.

Responsibilities/Typical Work Activities:

  • Directs all (MDS) assessments for the nursing home by determining schedules and staff responsibility;
  • Reviews and validates all MDS assessments;
  • Maintains records of MOS scores as a triple check against qualifiers;
  • Reviews all residents’ MDS assessments, ensuring completeness and accuracy and takes corrective actions as needed;
  • Reviews eligibility for Medicare, determines Medicare/HMO coverage status and completes necessary notification forms;
  • Assists registered nurses in the implementation and completion of care plans as needed;
  • Maintains records of all completed assessments;
  • Develops in-service training material on MDS, CAAs, care planning and related rules and regulations and conducts group and individual training sessions;
  • Schedules and conducts interdisciplinary team care conferences for each resident as required with nurse managers and other clinical staff;
  • Prepares a variety of records and reports;
  • Directs and supervises resident care

Full Performance, Knowledge, Skills, Abilities and Personal Characteristics:

  • Thorough knowledge of the rules, regulations and policies covering Medicare, Medicaid, HMO and Medicaid LTC and the eligibility requirements for these programs;
  • Thorough knowledge of professional nursing techniques and their relation to medical practices and skill in their application;
  • Thorough knowledge of resident needs in a long-term care facility;
  • Thorough knowledge of facility operations and the need to coordinate all services for the well-being of the residents;
  • Ability to secure and direct the cooperation of others;
  • Ability to direct, understand and carry out technical, oral and written instructions;
  • Physical condition commensurate with the demands of the

Minimum Qualifications (EITHER):

Graduation from a regionally accredited or New York State registered four-year college or university with a Bachelor’s degree in Nursing and four (4) years of experience in Minimum Data Sets (MDS) in skilled nursing facility.

OR

Graduation from an approved three (3) year school of professional nursing and seven (7) years of MDS experience in a skilled nursing facility.

OR

Graduation from a regionally accredited or New York State registered two (2) year college with an associate degree in nursing and ten (10) years of experience in MDS in a skilled nursing facility.

Special Requirement:

  • Eligibility for a license issued by the State of New York to practice as a registered professional
