Job summary

Ivy Terrace at Fullerton, part of Oakmont Senior Living, seeks a Resident Care Coordinator to coordinate resident care and staffing for a Sunday–Thursday 9:00am–5:30pm shift. This role engages with physicians, nurses, and facilities staff to support residents’ independence and wellbeing.

Qualifications

• Must be at least eighteen (18) years of age.
• High school diploma or equivalent.
• Prefer one (1) year of experience supervising and managing employees.
• Ability to effectively train staff.
• Flexibility in work schedule including holidays, evenings, weekends, and additional shifts.
• Proficient in MS Office (Word, Excel, Publisher, PowerPoint).
• Must pass Criminal Background check and Health Screening including COVID-19 PCR test within 72 hours of start date.

Responsibilities

• Coordinate resident care and staffing to meet residents' needs.
• Communicate with healthcare professionals, discharge planners, nurses, and state agencies as needed.
• Assist Health Services Director in coordinating care and arranging resident escorts as needed.
• Participate in the community’s resident assessment program per policy and state regulations.
• Recruit, develop, manage and schedule Care Providers and Medication Technicians.
• Maintain a safe and secure environment following safety standards.
• Encourage teamwork through cooperative interactions with staff and other departments.
