Job summary

Mile Bluff Medical Center in Wisconsin is seeking a Resident Care Coordinator to help coordinate resident care through MDS, participate in physician rounds, and support direct care staff.

The role requires an RN license in Wisconsin, a two-year nursing degree, and CPR certification; wound care certification is preferred. Weekend work is not required and holidays are paid.

Schedule: 80 hours per pay period; Monday - Friday - 8:00am to 4:30pm

Weekend Requirement: No weekends

Holiday Requirement: Paid holidays

Position Summary:

The Resident Care Coordinator position assists with coordinating resident care through completion of the MDS, rounding with physicians, care planning, being available to direct care staff as a resource and providing help when needed, coordinating individual programs in order to help provide comprehensive resident care and gain accurate financial reimbursement to the facility.

Position Responsibilities:

• Serve as the wound care coordinator.
• Coordinate resident care through completion of the MDS.
• Serve as the Restorative Program Coordinator.
• Oversee care plan meetings.
• Participate in physician rounds and assist direct care staff.
• Perform other duties as requested.

Position Requirements:

• Two-year degree in nursing required.
• Must hold and maintain Wisconsin Registered Nursing license.
• Wound care certification, RAC certification preferred.
• MDS certification a plus.
• Current CPR certification.
• 6+ months of related work experience required.
