Citizens Memorial Hospital Foundation (CMH) seeks a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to be our MDS Coordinator-Resident Care Coordinator for the Ash Grove Healthcare Facility. You will support residents through Medicare/Medicaid assessments and care planning, collaborating with the interdisciplinary team.

Qualifications

• Experience in LTC and MDS (MDS 3.0) assessments.

Citizens Memorial Hospital Foundation (CMH) is recruiting a RegisteredNurse (RN) or a Licensed Practical Nurse (LPN) to be our MDS Coordinator-Resident Care Coordinator to care for our residentsin our Ash Grove Healthcare Facility. CMH is made up of 34 primary and specialty clinics, 6 Long-Term Care facilities and 1 residential care facility. Offering acute care, emergency services, ambulatory care, pharmacies, walk-in clinics, surgeries, addiction recovery, therapies and more, CMH has its community covered!

A typical day includes:

• Completing, auditing, and submitting Medicare, Medicaid, and PPS assessments (MDS 3.0) according to regulatory timelines.
• Collaborating with the Interdisciplinary Team (IDT) to develop and update individualized, comprehensive care plans.
• Reviewing clinical records and nursing documentation

Job Requirements:

• Experience: LTC and MDS experience preferred
• Skills: Strong attention to detail
• License: LPN, LVN, RN
• Physical: Ability to sit or stand for extended periods, and move throughout the facility to monitor resident care.

The position is:full-time, days.
