Job summary

Trinity Community at Beavercreek is seeking a Staff Nurse (LPN) to coordinate care for about 25 residents in their unit, operating within scope of practice and following physician orders, community policies and procedures, and local, state and federal regulations.

You will perform assessments, administer medications and treatments, monitor vital signs, assist in care planning, and ensure safe, high-quality nursing care through thorough documentation and collaboration with the care team.

Responsibilities

• Manages resident treatments
• Distributes and administers prescribed medications and treatments, including IVs if certified
• Assesses residents, under RN supervision
• Participates in assessing, planning, implementing and evaluating resident care delivery
• Assists in completing the initial and ongoing nursing assessments needed to plan care and interventions
• Makes resident rounds to ensure care delivery meets quality standards and restorative protocols
• Observes vital signs and hydration status to detect deviations from normal limits
• Orders prescribed medications, requisitions supplies, equipment from appropriate resources
• Documents resident's responses to treatments/medications, progress toward care plan
• Interprets policies and procedures to personnel, residents, visitors, and government agencies as required
• Assists in completion of the care plans (acute and long term)
• Evaluates effectiveness of care interventions, identifies problems/trends and develops alternative interventions
