Resident Care Coordinator

The main function of the Resident Care Coordinator is to coordinate, lead, and direct the care and services provided to residents by nursing assistants, med techs and other professionals or paraprofessionals. May also provide direct resident care as required.

Your Responsibilities
  • Ensure all residents are treated with respect and dignity, recognizing individual needs and encouraging independence
  • Assist in the management of the healthcare of all residents within the community, including the dissemination of information to families and staff
  • Assist in medication management and care planning process as assigned
  • Assist the Director of Nursing or Executive Director in overseeing nursing staff - hiring, disciplines, scheduling, etc
  • Assist with facility procedures for admission, discharge and transfer of residents
  • Consistently work cooperatively with residents, residents’ representatives, facility staff, physicians, consultant personnel and ancillary service providers
  • Reports any issues or problems that may arise to the Director of Nursing or Executive Director
  • Assists families and other visitors as needed
  • Complies with state, federal, and all other applicable health care and safety standards
  • Promotes and exemplifies the Priority Life Care mission and values at all times
Qualifications
  • High School Diploma or equivalent; minimum of 3 years of previous long-term care experience; or an equivalent combination of education and experience
  • Must be licensed in the applicable state as a Licensed Practical Nurse (LPN) or Registered Nurse (RN)
  • Prior experience managing staff and working in a long-term and/or assisted living setting preferred
  • Experience with the applicable state Survey Rules and Regulations, Medicare, and Medicaid
