Job summary

Glenncrest at Buckhead in Atlanta, GA is seeking an experienced LPN to join as Resident Care Coordinator. You will collaborate with the Director of Wellness to oversee daily nursing care operations and supervise nursing staff, ensuring high-quality resident care.

The role requires coordinating care plans, staffing, onboarding, and maintaining compliance with state and federal guidelines. Flexibility for nights, weekends, and holidays is essential.

Qualifications

• Must have a caring heart and be willing to serve others.
• Current state LPN license.
• Two years of nursing experience.
• Current CPR certification.
• Bachelor's degree in Nursing from accredited nursing college or university (Preferred).
• Current First Aid certification (preferred).
• Experience in Assisted Living or Long-term care (preferred).
• Experience in dementia care (Preferred).
• Must be flexible and prepared to assist on nights, weekends and holidays.

Responsibilities

• Works with Director of Wellness to ensure effective and efficient daily operations within the wellness department.
• Maintain current task lists for caregivers and update timely as ADL changes occur.
• Schedule the care-plan dates according to state regulation guidelines.
• Fill in for Medication Tech or Caregiver on occasion.
• Work as a Manager-On Duty as directed by Executive Director.
• Handle staffing schedule to meet residents' needs, state requirements, budget and census while addressing shortages to ensure adequate staffing.
• Directly supervises nursing and personal care staff in the management of resident well-being and care.
• Assists with onboarding and training of new wellness staff.
• Builds a positive culture in the wellness department by actively providing care alongside team members.
• Assesses health, functional and psycho-social status of residents and proactively manages their care.
• Ensures superior care and promotes service excellence.
• Works with Director of Wellness to ensure community meets Federal and State guidelines and requirements.
