Job summary

The Pearl at Five Forks, part of Phoenix Senior Living, is seeking a Resident Care Associate (CNA) to provide hands-on care in assisted living and memory care settings. You will work under the Family Experience Director, supporting residents with daily activities and personalized care plans.

Responsibilities include implementing PSPs, monitoring hydration, assisting with meals, and ensuring safety and cleanliness across shifts. Weekend availability and flexibility are required.

Qualifications

• High School education preferred.
• Experience with elders or disabled individuals preferred.
• Must be at least 18 years old.
• Knowledge of Assisted Living and Dementia care preferred.
• Ability to work weekends, evenings and flexible hours.

Responsibilities

• Participates in developing and updating Personalized Service Plans.
• Follow hydration schedule for residents.
• Complete meal attendance sheets and arrange room service if needed.
• Respect resident rights and privacy.
• Initiate and assist with activities for residents.
• Check daily for cleanliness and condition, escalate issues as needed.
• Ensure safety: sanitation, universal precautions and hazard handling.
• Complete ADLs as per PSP and respond promptly to security calls.
