Job summary

Highland Park Rehabilitation & Care in Aurora, Colorado is seeking a compassionate Social Services Assistant to help residents transition into the facility and coordinate admissions for a smooth start.

You will provide emotional support, coordinate with interdisciplinary teams to develop care plans, and facilitate resident engagement while assisting with daily living tasks and maintaining detailed records. This role supports the wellbeing of residents and families.

Responsibilities

• Take the lead on providing an incredible admissions experience including orienting residents to the facility and organizing crucial aspects of their transition.
• Provide emotional support and companionship to residents, addressing their social, emotional, and spiritual needs
• Coordinate with interdisciplinary teams to develop and implement individualized care plans
• Facilitate resident engagement in activities, outings, and events, promoting socialization and overall well-being
• Assist with daily living tasks, such as personal care, grooming, and meal assistance
• Maintain accurate and detailed records of resident interactions, needs, and progress
• Collaborate with family members and caregivers to ensure seamless communication and support
• Participate in staff training and education programs to stay updated on best practices and regulatory requirements
