Job summary

Great Lakes Management in Minnetonka, MN is seeking a dedicated Resident Assistant to provide hands-on care and emotional support to residents in a long-term care setting. You will assist with activities of daily living, communicate with residents and families, and collaborate with the case manager to address changes in condition.

This role offers shift options (2:30p-10:45p, 6:30a-2:45p, 10:30a-7am), a pay range of $18.00–$24.00 hourly, location at 5300 Woodhill Rd Minnetonka, MN 55345, and

Qualifications

• Six months of experience in long-term care or assisted living (Preferred).
• Registered as a Nursing Assistant, Home Health Aide or Certified Nursing Assistant (Preferred).
• Ability to communicate with residents, families and care team.

Responsibilities

• Assist residents with activities of daily living (ADLs) as per service agreements.
• Support residents physically and emotionally; participate in dining and activities.
• Notify case manager of changes in condition or concerns.
• Complete required documentation daily and maintain communication books.

Education

Nursing Assistant Certification (CNA) or equivalent

Resident Assistant Job Objective(s):

The Resident Assistant is responsible for providing hands -on care, both physical and emotional, as outlined in each resident’s service agreement. R esponsible for effectively communicating with residents, families, and notifying the case manager of any changes in condition or concerns. The Resident Assistant assist s in the dining room and participating in activities and outings .

Resident Assistant Job Responsibilitie s :

• Assistresidents with ADL’s (Activities of Daily Living) asindicatedon the individualized service agreement
• Help each resident to reach andmaintainthe highest level of performance and independence.
• Provide residents with both physical and emotional support.
• Assistwith treatments as delegated by the Registered Nurse per the resident service agreement
• The Resident Assistantobservesclientsand reportschanges in physical and emotional condition such as change in attitude, loss of appetite, weight, etc.tothe casemanager.
• Interactwith residents to provide reassurance, engagement, and redirection as appropriate.
• Complete all required documentation daily, service schedule, flow sheet, communication book, etc.

Qualifications :

• Six months of experience in long term care or assisted living residence (Preferred)
• Registered as a Nursing Assistant, Home Health Aid or Certified Nursing Assistant (Preferred)
• Ability to make responsible choices and decisions and act in the resident’s best interests.
