Great Lakes Management Co. Bloomington, MN is seeking a Resident Assistant to provide hands-on care and supportive services to residents in a long-term care setting.

The role involves assisting with daily living activities and participating in engagement and activity programs. The incumbent will communicate with residents and families, coordinate with the case manager, and document daily care and conditions as part of the resident service plan.

Formación

• Six months of experience in long term care or assisted living (preferred).
• Registered as a Nursing Assistant, Home Health Aide or Certified Nursing Assistant (preferred).
• Ability to make responsible choices and act in the resident's best interests.

Responsabilidades

• Assist residents with ADLs as indicated by the service agreement.
• Help each resident reach and maintain the highest level of independence.
• Provide residents with physical and emotional support.
• Assist with treatments as delegated by the Registered Nurse per the service agreement.
• Observe clients and report changes in condition to the case manager.
• Interact with residents to provide reassurance, engagement, and redirection.
• Complete all required documentation daily, service schedule, flow sheet, and communication book.

Resident Assistant Job Objective(s):

The Resident Assistant is responsible for providing hands -on care, both physical and emotional, as outlined in each resident’s service agreement. R esponsible for effectively communicating with residents, families, and notifying the case manager of any changes in condition or concerns. The Resident Assistant assist s in the dining room and participating in activities and outings .

Resident Assistant Job Responsibilitie s :
