• Collaborate with the interdisciplinary team to develop personalized plans of care that incorporate recreational and therapeutic activities.
• Coordinate and facilitate activities, including arts and crafts, games, sports, and entertainment events.
• Assess resident needs and preferences to ensure activities are tailored to their abilities and interests.
• Maintain accurate records of resident participation and activity schedules.
• Monitor and evaluate the success of activity programs and make recommendations for improvement.
• Participate in resident assessments and care plan development as needed.
