Job summary

North Mississippi Health Services in Tupelo is seeking a patient care coordinator to help improve the flow of care in the Critical Care waiting area. You will coordinate care, provide information, and support families during stressful moments.

This role emphasizes communication, empathy, and collaboration with nurses, physicians, and support staff to ensure timely updates and smooth service for patients and visitors.

Responsibilities

• Effectively coordinates care to family members by monitoring and evaluating interdisciplinary care.
• Orients and educates patients, families, and visitors to the Critical Care waiting area, policies, and procedures.
• Greets individuals in a positive and caring manner.
• Relays the needs and concerns of patients' families to staff.
• Provides emotional support to family members as needed.
• Receives requests and provides general information to patients and family members.
• Assists in the resolutions of patient and family issues/problems.
• Promotes effective communication amongst care team, treating clinicians, patients, family members, etc.
• Serves as a liaison between patient care personnel and patients/families/visitors to ensure effective communication/interaction.
• Assesses and maintains waiting room supplies in a cost-effective manner.
• Adheres to NMHS/NMMC Policies/Procedures/Guidelines and complies with local/state/federal policies.

Coordinates

• Assists family with grieving processes and procedures Communication:
• Promotes effective communication amongst care team, treating clinicians, patients, family members, etc. Liaison:
• Serves as a liaison between patient care personnel and patients/families/visitors to ensure effective communication/interaction Inventory:
• Assesses and maintains waiting room supplies in a cost-effective manner
