Parkland Health seeks a highly qualified clinical pharmacist to lead integrated pharmacy services in an assigned area. You will educate, mentor, and collaborate with medical teams to optimize patient outcomes using evidence-based medication use.

Requirements include PharmD or Bachelor's in Pharmacy and completion of PGY1/PGY2 residencies or fellowship, with Texas licensure within 90 days. This role emphasizes leadership, patient safety, and cost-effective therapies within Parkland Health.

Qualifications

• Must have a Bachelor's Degree in Pharmacy or PharmD from an accredited school.
• Post-graduate year one (PGY1) residency training program and post-graduate year two (PGY2) residency training program or fellowship.
• Texas Pharmacist License or eligible for Texas Pharmacist License; licensure within 90 days of employment.

Responsabilités

• Evaluates, monitors, &/or modifies patient pharmacotherapy to ensure effective and safe patient care.
• Designs, implements, and manages patient drug regimens based on evidence based medicine and formulary compliance.
• Participates in cost effective, evidence based therapies or protocols and monitors drug expense.
• Acts on medication management committees and supports regulatory standards.
• Educates patients on medications and ensures understanding of drug usage and side effects.
• Provides mentoring and education to pharmacy students and residents.
• Attends journals clubs and in-services, providing feedback and updates.
• Documents interventions and ensures safety opportunities across the medication management process.
