Job summary

The position requires completing an ASHP-accredited PGY1 and PGY2 residency, Texas Pharmacist License or eligibility, and a strong background in renal care and medication therapy management.

Qualifications

• Bachelor's Degree in Pharmacy or Pharm.D. from an accredited school.
• ASHP-accredited PGY1 and PGY2 residencies or fellowship completed.
• Texas Pharmacist License or eligible; licensure within 90 days.
• Board certifications in ambulatory care, cardiology, critical care, geriatrics, infectious diseases, pharmacotherapy, oncology, or psychiatry preferred.
• Experience in the assigned practice area preferred.

Responsibilities

• Evaluates and optimizes patient pharmacotherapy to ensure safe and effective care.
• Collaborates with medical teams to design evidence-based regimens and improve outcomes.
• Participates in drug information inquiries and formulary reviews to support safe use of medicines.
• Educates providers and patients about therapies, monitoring, and potential adverse effects.
• Contributes to quality improvement, medication safety initiatives, and cost-effective therapies.
• Mentors pharmacy students and residents through case-based learning and education programs.
