Job summary

Qualifications

• Registered Dietitian with current licensure.
• Minimum 1 year of professional clinical nutrition experience.
• Renal experience preferred.
• Strong leadership, communication, and customer service skills.
• Proficient with computer systems and medical records.

Responsibilities

• Assess patients' knowledge of diet and kidney disease; provide education tailored to learning needs.
• Provide patient/family/caregiver education on nutritional status, labs, fluid status, and disease impact.
• Complete nutritional assessments and participate in interdisciplinary care plans.
• Develop individualized nutrition plans in collaboration with the care team per guidelines.
• Monitor nutrition therapy; document in medical records; adjust plans as needed.
• Review labs and trends; coordinate with team on interventions and referrals.
• Educate and coordinate Pre-ESRD education as appropriate; liaise with Case Manager as needed.
