Job summary

U.S. Department of Veterans Affairs in Pittsburgh seeks a Registered Nurse – Renal Case Management to support the Medicine Service Line at the University Drive Campus.

The RN will ensure care is delivered per standards and collaborate with the Renal team to develop patient-driven care plans and program improvements. The role covers clinical and administrative duties, with a focus on care coordination, patient advocacy, and adherence to VA and regulatory guidelines.

Responsibilities

• Ensure patient care is provided per standards and Medical Center policies.
• Provide care in a safe, efficient, timely manner within Renal service line.
• Monitor quality measures and stay current with VA, Joint Commission, state and local standards.
• Act as patient advocate and coordinate care with the Renal team and RCIs.
