Job summary

Responsibilities

• Provide lifestyle coaching in tobacco cessation, weight management, sleep hygiene, stress, nutrition, physical activity, and maternal health.
• Oversee a panel of members, assess needs, and triage to resources as needed.
• Create care plans with long/short term goals and regular follow-up contacts.
• Demonstrate clinical outcomes and cost savings from wellness interventions.
• Document all activities in compliance with regulatory and internal standards.
• Participate in workgroups to improve efficiency and member-centric wellness services.
