Job summary

Northwestern Medicine is seeking a remote RN Case Manager (PT) to support Utilization Review at Catherine Gratz Griffin Lake Forest Hospital. This role provides staffing coverage with no set schedule and requires availability on weekdays and weekends; shifts are 6a–6p or 6p–6a.

You will develop care plans, coordinate transfers, document CMS-compliant reviews, and educate staff to optimize patient outcomes while maintaining quality and regulatory standards.

Qualifications

• Registered Nurse with an IL license.
• BSN degree required.
• Minimum 3 years of related clinical experience.
• MSN preferred.
• ACM certification preferred.
• Membership in a professional nursing organization preferred.

Responsibilities

• Develops individualized treatment plans for patients in collaboration with the patient and family.
• Coordinates and leads care conferences for patients with longer stays.
• Documents and refers as needed to risk management and quality departments.
• Assesses discharge needs and coordinates transfer to appropriate care levels.
• Performs chart audits and ensures CMS and regulatory compliance.
