Job summary

Spring Health is seeking an independently licensed mental health professional to join the Care Navigation team in a fully remote role. The Trauma Clinical Care Navigator will assess, refer, and coordinate care for PTSD and complex trauma, reporting to the Lead Specialty Care Navigator.

The position runs Monday–Friday, 9–5 PT, with a focus on high-touch member support across multiple care stages. The role requires LCSW/LMFT/LPC, 2+ years of PTSD/trauma experience, and strong skills in

Qualifications

• Licensed mental health professional (LCSW, LMFT, or LPC) with active license.
• Minimum 2+ years of clinical care experience with PTSD and complex trauma.
• Experience with motivational interviewing, diagnostic assessment, and triage.
• Knowledge of phased-based trauma treatment and PTSD EBPs (PE, CPT, EMDR, ART).
• Excellent listening and written/verbal communication; commitment to inclusion.

Responsibilities

• Telephonic screening, brief intervention, and referral to trauma providers.
• Apply active listening, psychoeducation, and motivational interviewing to help members set goals.
• Utilize trauma treatment model to provide stabilization and engagement in care.
• Coordinate care with Spring Health providers and document in the EHR.
• Interact with care teams to ensure smooth member experience; participate in trainings.
