Job summary

Mass General Brigham in Somerville, MA, is seeking a Transitions of Care Clinical Pharmacist to support Medicare, Medicaid and Commercial risk patients as they move from hospital to home, using telehealth to engage patients remotely.

You will conduct post-discharge medication reconciliation, identify drug-related issues, and optimize therapy with provider collaboration to reduce readmissions and improve outcomes.

Qualifications

• Pharm D degree from an accredited college/university.
• Massachusetts Pharmacist license required.
• Board certification and/or PGY1 residency required.

Responsibilities

• Review discharge summaries and conduct remote consultations for post-discharge reconciliation.
• Review and analyze medication orders and patient profiles to resolve issues.
• Develop and implement medication therapy plans to optimize outcomes and adherence.
• Provide education on medication management via motivational interviewing.
• Assess adherence issues and develop plans to improve adherence.
• Address affordability concerns and simplify regimens where possible.
• Document patient encounters and update medication lists in EMR.
• Collaborate with multidisciplinary teams for coordinated patient care.
• Participate in drug therapy management under collaborative agreement.
• Contribute to quality improvement and best practices in transitions of care.
• Maintain professional competency through ongoing education.
• Collaborate with colleagues on related duties and projects.
