Job summary

Libertana Home Health is seeking a Transition Coordinator to coordinate housing and transition options for SNF residents across Los Angeles County and surrounding areas. This fully remote role supports the CA state liaison work and represents the Lead Organization for multiple counties.

You will pre-screen residents, coordinate ICP and TAR submissions, arrange RN visits, and ensure compliant care planning and billing. Strong communication and organizational skills are essential for success.

Qualifications

• Bachelor’s degree in healthcare, business administration or related field preferred.
• Microsoft Word, Excel, PowerPoint and Access proficiency required.
• Health care experience preferred.
• Experience with developing reports, newsletters, brochures, statistics and information analysis.
• Experience as a Community Liaison in the Los Angeles community.
• Must be at least 18 years old.
• Excellent written and verbal communication.

Responsibilities

• Coordinate housing and transition options for SNF residents.
• Liaise between California state agencies and local communities.
• Pre-screen and interview residents to identify needs.
• Contact SNFs monthly prior to discharge for documentation.
• Arrange RN visits to assess residents and create ICP.
• Prepare and submit ICP and TAR documentation.
• Coordinate DME and assistive devices.
• Maintain contact with SNFs and residents on care plans.
• Apply for waivers (ALW/NF/IHSS) when needed.
• Assist transition back into the community.
• Ensure billing timekeeping and reporting.
• Maintain compliance with HIPAA and confidentiality.
• Attend mandated trainings.
• Drive as needed within service area.
