The nurse will utilize telehealth technologies to monitor patient progress, provide education, and support self-management of chronic conditions, thereby reducing hospital readmissions and improving quality of life. This role requires strong clinical judgment and the ability to respond promptly to patient needs, emergencies, or changes in condition remotely. Minimum Qualifications:

• Current and unrestricted Registered Nurse (RN) license in the United States.
• License in a compact state with preference of additional licenses in California and Illinois.
• Minimum of two years of clinical nursing experience, preferably in case management, chronic disease management, or telehealth.
• Proficiency with electronic health record (EHR) systems and telehealth technology platforms.
• Strong communication skills to effectively engage with patients and interdisciplinary teams remotely.
• Ability to work independently in a remote environment while managing multiple patient cases efficiently.

Preferred Qualifications:

• Certification in Case Management (CCM) or Telehealth Nursing (CTN) is highly desirable.
• Experience working in a remote or virtual care setting.
• Background in managing complex chronic conditions such as diabetes, heart failure, or COPD.
