Job summary

Qualifications

• RN license required or eligible within licensure rules
• BSN preferred if hired; BSN must be pursued for newer hires
• BLS certification required (AHA or ARC) for healthcare providers

Responsibilities

• Coordinate pre-visit, during visit, and post-visit care planning
• Provide virtual nursing care and support to patients and providers
• Process patient referrals and care plan needs as requested
