Job summary

Optum Home & Community Care is seeking a National On Call Advanced Practice Clinician to provide telehealth care across settings. This remote role demands strong clinical judgment, collaboration with physicians, and efficient use of EMR for documentation during all shifts.

The position supports 24/7 coverage, with holiday rotas and premium pay for holiday work, offering flexible scheduling from anywhere in the U.S.

Qualifications

• Graduate of an accredited MSN or DNP program with NP certification.
• Active and unrestricted NP licensure in residence state and required locations.
• DEA license or ability to obtain post-hire per state regulations.

Responsibilities

• Provide telephonic/telehealth care and direction to patients 24/7.
• Deliver acute care, manage medical and behavioral conditions with physicians.
• Utilize EMR to document encounters and ensure CMS-compliant notes.
• Coordinate care across care settings and communicate with families.
