Job summary

Urrly is seeking a Telehealth Nurse Practitioner in Clark County, Washington to conduct Comprehensive Health Assessments via telehealth, review medical histories, and document risk adjustment findings.

As a fully remote 1099 contractor, you will help close quality gaps, support value-based care programs, and deliver clear care plans with thorough documentation; a WA NP license and Medicaid enrollment are required, with at least 24 hours per week.

Qualifications

• Active Washington NP license required.
• Must be registered/enrolled with Medicaid.
• At least 1 year of Family Medicine or Internal Medicine experience.
• Experience caring for Medicare and Medicaid populations.
• Familiarity with HEDIS and risk adjustment workflows.
• Ability to work at least 24 hours per week.
• Comfortable delivering care virtually and documenting thoroughly.

Responsibilities

• Conduct Comprehensive Health Assessments through telehealth.
• Review medical history, medications, preventive care needs, and relevant member information.
• Document HCC risk adjustment findings accurately.
• Identify and help close HEDIS quality measure care gaps.
• Use ICD-10 and CPT II codes to support complete documentation.
• Deliver clear care plans and follow-up guidance.
• Complete timely documentation supporting quality and value-based care programs.
