Overview

A leading healthcare client is seeking Remote Registered Nurses to support a new program that reviews health risk assessments and supports HEDIS gap closure. In this role, you will conduct inbound and outbound calls to assess patients, gather health history, and provide guidance based on physician-approved triage protocols. You will use your clinical judgment to help patients navigate the next steps in their care. This is a remote position offering the opportunity to make a meaningful impact in a healthcare program focused on patient care and well-being.

Responsibilities
  • Take inbound and make outbound calls to assess patients and gather health history.
  • Provide concise, supportive patient education during calls.
  • Record all phone call details using Athena encounter templates and text macros.
  • Use a physician-approved triage protocol to advise patients on whether to follow up with their provider, seek same-day care, or go to the emergency department.
  • Utilize strong clinical judgment to ensure patients take appropriate next steps in their care.
  • Complete a brief historical intake and document symptoms.
Qualifications and Skills
  • Minimum of 2+ years’ experience working as a Registered Nurse.
  • 1+ year of experience in telehealth.
  • 1+ year in care management/HEDIS/Stars, women’s health, or imaging navigation.
  • Active Registered Nurse (RN) Compact License and RN License in one or more of the following states: CA, IL, NY, MN, MI, OR, NV, AK, CT, HI.
  • Electronic Medical Record (EMR) experience; Athena EMR experience preferred.
  • Experience handling a high volume of medical phone calls; tech-savvy with ability to learn new platforms (including Slack and Google Suite).
  • Strong clinical judgment and ability to work independently; excellent communication skills for patient education.
  • Proven ability to manage and prioritize patient cases based on urgency while working with diverse populations.
  • Bilingual in Spanish (preferred).
  • Experience in Women’s Wellness, Dialysis, Oncology is highly preferred (e.g., Breast Cancer Screening HEDIS BCS, Osteoporosis Screening in Women HEDIS OSW).
  • Preferred Zendesk VoIP or similar computer-based calling software; knowledge of HEDIS or Risk-Based Assessment concepts.
