Job summary

Catholic Charities of St. Paul and Minneapolis is seeking a Social Worker I to join the Aging and Disability Services team.

The role supports older adults and individuals with disabilities, delivering trauma-informed, client-centered case management with in-home visits and community referrals. Qualifications

• Bachelor’s or Master’s degree in social work, psychology, sociology, human services, or related field preferred.
• Pass DHS background check and meet required standards for case management.
• Valid driver’s license and proof of current insurance coverage.
• Experience with diverse populations and ability to engage in community outreach.

Responsibilities

• Provides initial and annual assessments and case management including in-home visits.
• Develops service plans and connects clients with Catholic Charities programs and community resources.
• Advocates for clients to obtain services and benefits; assists with required paperwork.
• Maintains complete case documentation and adheres to funder/licensure requirements.
• Coordinates with supervisor and team; uses trauma-informed, person-centered approaches.
