Job summary

Parallon is seeking a Core Measures Data Abstractor II to perform sepsis data abstraction from medical records for facilities within a 60‑mile radius of a HCA facility. You will work from home and use systems such as COMET, TheraDoc, NHSN to ensure accurate reporting.

The role requires HIM experience and healthcare coding/nursing education. RHIT/RHIA/CCS certifications are strongly preferred; RN/LVN credentials are a plus.

Qualifications

• 2+ years of experience in Health Information Management; Coding, Nursing, and/or Health Registry abstraction experience required
• Completion of a certified coding or nursing program strongly preferred
• RHIT, RHIA, CCS certification strongly preferred
• LVN or RN preferred
• Undergraduate degree in a healthcare related field required. Extensive experience (5 years or more) may be considered in lieu of formal education.

Responsibilities

• Completes abstraction process for assigned facility(ies), including abstraction of cases into the required system (e.g., COMET, TheraDoc, Digital Innovations, NHSN, etc.)
• Reviews medical records to abstract information according to standards of regulatory and accreditation agencies (CMS, TJC, NHSN)
• Performs timely abstraction to ensure compliance with standards
• Completes edit checks and makes appropriate changes on a timely basis
• Follows standards and Parallon instructions to abstract all reportable cases
• Assists with case follow-up as requested
• Attends educational activities as approved by Manager or Director
• Maintains clinical knowledge of various abstracted measures
• Communicates in a timely manner with manager to achieve measure compliance
• Submits data timely through the appropriate reporting system
• Resolves errors resulting in the rejection of records from the data entry system
