Job summary

HCF is seeking a Registered Nurse Assessment Coordinator (RNAC) for a PRN remote role. The RNAC coordinates MDS and Care Planning with the Interdisciplinary team across multiple locations, ensuring timely and accurate submissions. Experience with MDS timing, electronic submissions, and QA processes is essential.

The position requires PA and OH RN licenses and is based in Greensburg, PA, with remote work options and up to 40 hours per week.

Qualifications

• Coordinate the MDS and Care Planning process with the Interdisciplinary team.
• Must precheck and electronically submit MDS to State agency and review validation reports.
• Schedule and timely completion of MDS and Care Plans with Case Manager and team.

Responsibilities

• Assist with nursing staff education in documentation and MDS processes.
• Complete MDS assessments within established timeframes for admissions, quarterly, annual, and significant change.
• Coordinate submission of MDS and Care Plans with the team and Case Manager.
• Participate in the QAPI process.
• Access, print and evaluate Quality Indicators.
