Start Date: 6/30/2025
Pay Rate: $2000.00 - $2700.00

Position Summary: Remote RN - Home Health & Poly-Chronic Disease Management

Position Duties: Assist Poly Chronic patients with medication review, care plan development, disease process education, coordination of care, making appointments, etc.

Minimum Qualifications: 2+ years RN experience. Active and unencumbered RN licensure in the state of assignment or compact licensure where applicable.

Preferred Qualifications: Experience with poly chronic disease management, CKD, CHF, and related dynamics. Ability to make recommendations and educate patients on health-related choices. Experience with in-home care management and in-home dialysis is strongly preferred.

Length of Assignment: 26 weeks

Shift / Hours per Week: M-F

Systems: Salesforce

Start Date: 6/30/25
