• 1+ year of SharePoint experience.

Responsibilities

• Lead clinical performance improvement projects for Medicare Provider Collaboration groups.
• Establish trusting relationships with provider partners and promote communication.
• Serve as clinical expert on CM, gap closure, and workflow discussions.
• Collaborate with engagement managers and data teams on Stars/quality initiatives.
• Coordinate and conduct clinically focused meetings to prioritize goals.
