Job summary

3:15 is looking for a qualified Registered Nurse (RN) to manage the care of patients through various continuous care programs including Remote Patient Monitoring. The RN will welcome patients, educate them on health device usage, and create personalized care plans. Experience with adult health is preferred.

Qualifications

• Active and Unrestricted Georgia RN License required, Compact License preferred.
• At least three years of experience in adult health preferred.
• Comfortable working remotely but collaboratively.

Responsibilities

• Welcome patients into continuous care programs and review benefits and services.
• Create a personalized care plan with patients via phone or video visits.
• Connect frequently with patients to review care plans and device readings.

Skills

Excellent communication skills (oral and written)

Proficient knowledge of Electronic Medical Records (EMR)

Problem-solving skills

Ability to multi-task

Teamwork skills

Education
