Qualifications

• 5+ years of experience as a Registered Nurse (RN) with at least 1 year in a hospital setting.
• RN must hold an active multistate license via the Nurse Licensure Compact (NLC) or equivalent state licenses for each state supported.
• 1+ years of experience documenting electronically.
• 1+ years of experience in Oncology, Transplant, Specialty Pharmacy, Pediatrics, Medical/Surgical, Behavioral Health/Substance Abuse or Maternity/Obstetrics.

Responsibilities

• Telephonic case management with patients and care teams for fully and/or self-insured clients.
• Apply clinical guidelines, care management plans, policies, procedures, and regulatory standards while assessing benefits and patient needs.
• Use clinical judgment to implement strategies to reduce risk factors and address complex health indicators.
• Assess information from multiple sources to address all conditions affecting functionality.
• Escalate cases and discuss with supervisors/care conferences to support goals and results.
• Collaborate with clinical colleagues to overcome barriers and meet objectives.
