Job summary

Jobtailor is seeking a Registered Nurse with case management expertise to support dual-eligible Medicare and Medicaid members from a home-based setting in Illinois. The role requires collaborating with members, providers, and community organizations to address health care and social determinants of health while maintaining HIPAA compliance.

The successful candidate will exercise critical thinking, manage multiple systems, and coordinate appointments and benefits access.

Qualifications

• Must reside in the state of Illinois.
• Must live in or near North Cook County or South Cook County and surrounding areas in Illinois.
• Must possess reliable transportation and be willing to travel up to 50-75% of the time from home location in and around Cook County and surrounding areas in Illinois.
• Minimum 3-5 years clinical practical experience.
• Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually.
• Excellent analytical and problem-solving skills.
• Effective communication, organizational, and interpersonal skills.
• Ability to work independently.
• Proficiency with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint, plus proprietary applications.
• Active and unencumbered Registered Nurse license in the state of Illinois.
• Direct/hardwired internet connection to a modem/router within 7 feet of the computer, with minimum 25 Mbps download and 3 Mbps upload; WiFi and satellite internet are not permitted.
• Quiet, secure, private designated home work location compliant with CVS Health and HIPAA guidelines.
• Must work within the state and city where currently living.
• Must provide own workspace furniture.
• Must obtain required internet speeds and adhere to Work From Home requirements if hired.
• Comfortable setting up and using multiple monitors and navigating multiple applications simultaneously.
• Ability to communicate via email, calendar invites, Teams messaging, and virtual meetings.
• Experience with Microsoft Office 365 or similar Google Workspace applications.
• Ability to log in to secure systems such as VPN and EHR portals, lock computer screens, manage strong passwords, and recognize suspicious emails or links.
• Ability to resolve common technical issues independently and contact IT for unresolved issues.
• Openness to learning new skills as the workplace evolves.
