Job summary

UnitedHealthcare is seeking a community health–oriented Registered Nurse to assess and connect members with resources across the care continuum in Hidalgo County, TX. You will work remotely within a defined territory, coordinating with providers, patients, and families to improve health outcomes.

Qualifications

• Current unrestricted Texas RN license.
• 2+ years of experience in a community health or healthcare setting.
• Intermediate level of proficiency with Microsoft Office applications.
• Reliable transportation, a valid driver, and ability to travel within assigned territory.

Responsibilities

• Assess, plan, and implement person-centered strategies.
• Connect members with financial, psychosocial, community, and state resources.
• Coordinate care across the continuum and serve as a key point of contact.
• Advocate for members and families to ensure their needs and preferences are heard and supported.
• Collaborate with providers, members, caregivers, and community partners to improve health outcomes.
