UnitedHealth Group is hiring a field-based Registered Nurse to deliver patient-centered care in Hidalgo County, TX and surrounding communities. You’ll split time roughly 50% in the field visiting members at home or in facilities, with opportunity to work remotely under our Telecommuter Policy.

Requirements include an unrestricted Texas RN license, 2+ years in community health, reliable transportation and internet at home, and proficiency with Microsoft Office.

Qualifications

• Current unrestricted Texas RN license.
• 2+ years in community health or healthcare.
• High-speed internet at residence.
• Reliable transportation and a valid driver’s license.
• Intermediate proficiency with Microsoft Office.

Responsabilités

• Assess, plan and implement care strategies individualized per patient.
• Identify and initiate referrals for social and state services.
• Manage the care plan as a single point of contact.
• Communicate health information to stakeholders for coordinated care.
• Advocate for patients and families to support needs and choices.
