Allina Health is seeking a Registered Nurse for the centralized anticoagulation program. This 0.6 FTE role offers fully remote work with orientation and occasional onsite requirements.

You will perform triage, refills, and coordination within outpatient and home care services, supporting patient-centered care across the team. The position emphasizes nursing diagnoses, planning, and evaluation while aligning care with evidence-based practice and professional standards.

Qualifications

• RN with Minnesota licensure or eligibility.
• Minimum 3 years RN experience preferred.
• Experience with anticoagulation programs or remote nursing support is a plus.
• Proficient in basic computer software and documentation.

Responsibilities

• Provide patient care support for centralized nursing program and anticoagulation activities.
• Triage, refills, and coordination of care in outpatient setting.
• Document care and collaborate with health care team to meet patient needs.
• Work remotely with occasional onsite orientation and coverage requirements.
