Minimum Requirements:

• Education: Required - Registered nurse diploma. Preferred - Associate's degree in nursing.
• Work Experience: Required - 1 year of hospital-based patient care. Preferred - Experience in Case Management or Utilization Review.
• Certifications: Required - Current RN license in state of practice. Preferred - CCM, CPUR, CPUM, or CPHM certifications.

Knowledge, Skills, and Abilities:

• Proficiency with computers and web applications.
• Effective communication skills.
• Knowledge of business/finance.
• Flexible work schedule and ability to travel as needed.

Key Responsibilities:

• Coordinate hospital admissions and transfers, ensuring smooth patient flow.
• Build professional relationships with healthcare teams and external entities.
• Manage patient care throughout their healthcare journey.
• Interact with payers to obtain financial certification and minimize denials.
• Educate patients and families about program benefits and services.
• Ensure compliance with laws and standards, maintaining confidentiality and ethical conduct.

Physical demands include exerting up to 50 pounds of force, exposure to blood and body fluids, and working in healthcare environments with occupational risks.
