This role involves determining the psychosocial, environmental, and family economic dynamics affecting the patient. Acts as a liaison and communicator with patients, caregivers, multidisciplinary team members, post-acute care, and third-party payers. Confirms care setting orders by physicians and reviews admissions and levels of care using hospital-approved criteria. Documents reviews in the designated data set.
