Job Summary

This role involves determining the psychosocial, environmental, and family economic dynamics affecting the patient. Acts as a liaison and communicator with patients, caregivers, multidisciplinary team members, post-acute care, and third-party payers. Confirms care setting orders by physicians and reviews admissions and levels of care using hospital-approved criteria. Documents reviews in the designated data set.

Qualifications

Education

• Required: Registered nurse diploma.
• Preferred: Associate's degree in nursing.

Work Experience

• Required: 1 year of hospital-based patient care experience.
• Preferred: Experience in case management or utilization review.

Certifications

• Required: Current RN license in the state of practice.
• Preferred: CCM, CPUR, CPUM, or CPHM certifications.

Knowledge, Skills, and Abilities

• Proficiency in computers, software, and web applications.
• Effective communication skills for diverse audiences.
• Knowledge of business and finance.
• Flexibility to work various shifts and travel between facilities.

Job Duties

• Coordinate admissions and transfers, ensuring smooth patient flow.

• Manage patient care coordination and education regarding rehabilitation services.
• Ensure compliance with laws, standards, and regulations.

Physical and Environmental Demands
