Season Health, a remote-first nutrition care company, is seeking a licensed RD/RDN to deliver Medical Nutrition Therapy to patients nationwide. You’ll conduct virtual assessments, develop care plans, and collaborate with physicians and health coaches to support coordinated care.

This role emphasizes ongoing education, evidence-based practice, and participation in care innovations while maintaining credentials and delivering high-quality patient-centered care from a fully remote setting.

Qualificações

• Current RD or RDN credentialed by the Commission on Dietetic Registration in good standing.
• Master’s Degree preferred; OR Bachelor’s Degree with RD/RDN status before the Master’s requirement (Jan 1, 2024).
• Excellent communication skills and a strong commitment to patient-centered care.
• Comfort using digital tools and platforms for virtual care delivery.
• Ability to work effectively with diverse patient populations and health conditions.
• Strong problem-solving, critical thinking, and organizational skills.

Responsabilidades

• Deliver high-quality, person-centered Medical Nutrition Therapy (MNT) for patients with chronic and complex conditions.
• Complete virtual assessments and develop individualized nutrition care plans.
• Write nutrition prescriptions tailored to clinical diagnoses and promote sustainable behavior change.
• Document all patient interactions in the EHR clearly and promptly.
• Collaborate with care teams to support coordinated patient care.
• Participate in content development for education, engagement, and outreach.
• Engage in continuous quality improvement and care-delivery innovation.
• Apply current nutrition research and evidence-based practices in patient care.
• Maintain required credentials, licensing, and ongoing education.
• Participate in cross-functional meetings and strategic projects when appropriate.
