Are you an organized, relationship-driven professional with a passion for connecting patients to the care they need? Gig Harbor Health and Rehabilitation is seeking a Remote Referral Manager to coordinate and manage our incoming patient referrals while building strong partnerships with hospitals, case managers, and healthcare providers — all from the comfort of your home!

About the Role:

As our Remote Referral Manager, you will serve as a critical liaison between referring partners and our admissions team. Your ability to streamline communication, ensure timely responses, and support efficient admissions will directly impact our ability to deliver exceptional care to those who need it most.

Responsibilities Include:

  • Manage and respond to all incoming referrals via email, fax, and electronic portals
  • Coordinate closely with the admissions and clinical teams to determine appropriateness for admission
  • Build and maintain strong relationships with hospital discharge planners, case managers, and physician offices
  • Track and report referral activity, conversion rates, and outreach performance metrics
  • Ensure all referral documentation is accurate and completed in a timely manner


What We're Looking For:

  • Proven experience in admissions, referral management, marketing, or case coordination (preferably in skilled nursing or post-acute care)
  • Excellent communication and organizational skills
  • Comfort working independently in a remote environment
  • Ability to multi-task and manage deadlines effectively
  • Tech-savvy with knowledge of EMRs and referral platforms (e.g., PointClickCare, naviHealth, Allscripts a plus)
