Job summary

Equip Health is seeking a dedicated Peer Mentor to join their multidisciplinary care team. This role is crucial for helping patients integrate into treatment by addressing barriers and providing necessary tools for recovery. The Peer Mentor will deliver skills-based sessions, monitor adherence to care plans, and build relationships with patients.

The ideal candidate has lived experience with eating disorder recovery, at least 2 years in patient care, and strong communication skills. The position requires commitment to provide 33 hours a week of direct patient care and participate in team meetings.

Qualifications

• Lived experience with eating disorder recovery in a solid space.
• At least 2 years of experience working directly with patients.
• Ability to be accountable for patient care plans.

Responsibilities

• Provide skills-based sessions to patients and caregivers.
• Monitor adherence to care plans and engage with patients.
• Participate in multidisciplinary treatment team huddles.
