Formación

• Active RN License (TX/Compact) required.
• Active CPR certificate required.
• Home Health experience, specifically geriatric home health care.
• Self-starter with strong organizational skills.
• Compassionate care approach in home health setting.
• Valid driver’s license required.

Responsabilidades

• Deliver skilled nursing care in patient’s home per physician orders and care plan.
• Collaborate with the care team to implement and update personalized care plans.
• Administer nursing treatments and manage clinical activities.
• Monitor patient progress and communicate findings to the team, patient, and family.
• Perform additional duties as needed.

Conocimientos

Active RN License (TX/Compact)

CPR Certification

Geriatric home health experience

Self-starter
