Qualifications

• Active Tennessee pharmacist license in good standing.
• PharmD or BS in Pharmacy degree.
• Able to make clinical decisions independently.
• Able to prioritize tasks and manage multiple projects.

Responsibilities

• Clinical evaluation and determination of UM requests, including denial language and program criteria.
• Direct consultation with healthcare providers on non-formulary and coverage issues.
• Collaborate with team to ensure consistency of clinical decisions across pharmacists.
• Other duties as assigned.
